What is Kentucky's 1915(i) RISE Initiative?

The 1915(i) RISE Initiative is a Kentucky Medicaid program that helps adults with serious mental illness (SMI) live independently and thrive in their communities. It offers home and community-based services (HCBS) that support recovery, reduce reliance on institutional care, and promote quality of life.
Who It’s For
This program is designed for Kentuckians 18 and older who:
Are enrolled in Kentucky Medicaid
Have a primary diagnosis of serious mental illness (like schizophrenia, bipolar disorder, major depression)
May also have a co-occurring substance use disorder
Experience challenges with daily living or community participation
What It Does
RISE provides personalized support based on each participant’s goals through a Person-Centered Service Plan developed with a case manager. Services are tailored to individual needs and may include:

✅ Help finding and keeping housing
✅ In-home independent living supports
✅ Medication management
✅ Support for education or employment
✅ Transportation to services and community activities
✅ Assistive technology
✅ Short-term respite for caregivers
✅ Supervised residential care (when needed)
How It Works
Referral: You or a provider refers to the program.
Screening & Assessment: A specialist reviews eligibility.
Planning: If eligible, you work with a case manager to create a service plan.
Service Delivery: Approved services begin based on your goals and needs.
